Do you see an Neurologist yearly? Your bladder issues sound to me like you may be experiencing spinal cord tethering. This very commonly causes changes in bowel and bladder function, sensation, etc.... I've been there.
Before I would let your Urologist do anything drastic like surgery I would CERTAINLY go to your Neurologist and be checked for tethered cord. I too have spina bifida so am very familiar with the bladder and bowel changes that come with tethered cord. NO fun. :(
